When I called I wasn't hopeful that I would be able to much help from customer service. But boy was I wrong!! The customer service rep that I spoke to, Kat L., was very courteous, friendly and very helpful!! She understood exactly what my problem was and made suggestions to reconcile the issue, sent the partially filled out form I needed to finish and how to finish form, and told me what else she needed me to send back to her.
